PHI Air Medical Closes Red Bluff Base After Payment Dispute with Elevance Blue Cross Licensee

PHI Air Medical, an Arizona-based air ambulance operator, ceased operations at its Red Bluff Municipal Airport base as of Monday, September 14, 2026, following a payment dispute with Elevance, a Blue Cross and Blue Shield Association licensee operating under the Anthem Blue Cross and Blue Shield brand across 14 U.S. states including California. Paul Julander, co-president of PHI Health, said this marks the first time PHI has closed a local base over an insurance reimbursement dispute. The company will continue serving Red Bluff-area patients from its Redding Regional Airport base, though Julander acknowledged this will cause some delays.
Julander attributed the dispute to the federal No Surprises Act, which took effect in 2022 to shield patients from unexpected out-of-network medical bills, including air ambulance costs. The law prohibits air ambulance providers from billing patients for the gap between insurer payments and total charges, requiring insurers and providers instead to settle differences through a federal arbitration process; critics have raised concerns about the process's complexity and potential payment delays to providers.
Julander said smaller bases like Red Bluff face greater financial exposure because they handle fewer transport requests, leaving them more vulnerable when insurers fail to meet reimbursement obligations. The Red Bluff base, which began operations in 2024, transported hundreds of patients during its time in service. PHI operates 24 hours a day, seven days a week, responding to calls from fire departments, law enforcement agencies, and hospitals.
